What is Community Connections?
GROUP HOMES / RESIDENTIAL / DAY SUPPORTS

Group Homes

A warm, supportive environment is provided in 12 group homes in Brantford and Brant County. The physical space requirement for this population is larger than usual because of the size of the residents' wheelchairs and the requirement of alternative positioning systems and other health support equipment. Therefore, homes are adapted accordingly.

Group Homes
The adaptations required for an appropriate environment include mechanical lifts, extra wide doorways and hallways, and bathrooms with raised bathtubs, change tables and ceiling mounted lifts. The specialized environment required is not presently available in the community and is most cost efficient to obtain through new development rather than renovation.

Residential

Each individual's wishes regarding support needs is determined through an Essential Life Plan (ELP).  The ELP is developed with the individual supported, family members and Brantwood staff.  The ELP is a working document that continuously evolves and changes as the wishes and needs of the individual change.

Residential
Residential supports provide opportunities for participation in the activities of daily living such as cooking, cleaning, shopping, social/recreational and religious activities, etc.

Day Supports

With the assistance of dedicated, caring staff, the individuals supported by Brantwood participate in all aspects of community living, such as shopping, cooking, holidays, housework, church, etc.
